Breast Implants Incision Techniques?

A breast augmentation procedure usually lasts one to two hours and is typically performed with general anesthesia. To minimize scar visibility, incisions are made in inconspicuous places. Therefore, they may be located:

  • In the crease on the underside of the breast (commonly called inframammary),
  • Around the areola, the darker skin around the nipple (commonly called periareolar), or
  • In the armpit (transaxillary and not as commonly used)

The breast in then lifted and a pocket is created, where the implant is inserted, hence, below or above the pectoralis muscle. Breast augmentation FAQs

Breast Implant Placement Options; above or below thew muscle?

Most breast implants are placed under the muscle because:

  • This provides additional tissue thickness to better conceal the edges of the implant in the upper breast,
  • Lowers the rick of capsular contracture,
  • Resists tissue stretching somewhat better over the period of time,
  • Allows for better quality mammograms,
  • Results in a more natural bust line.

This a 29 year patient from San Diego who was interested in breast augmentation. She had a consultation with Dr. Jaibaji at his Coronado office. During the consultation, her goals were evaluated and she elected to have Mentor silicone breast implants.

The implants were placed under the muscle. The incision was made around the areola.  425 cc.

The implants were placed under the pectoral muscle. The incision that was used was placed in the lower aspect of the areola. The after photos shown are two months after the surgery. Her cup size went from 32-A to 32-D.

Breast Implant types; Saline, Silicone and Gummy Bear

Saline Implants

Saline implants are filled with sterile salt water. They’re inserted empty, and then filled once they’re in place. Since they are inserted empty, they require a smaller incision size. Saline breast implants are available to women 18 and older for breast augmentation and to women of any age for breast reconstruction. They are less expensive compared to silicone implants.

Silicone Implants

Silicone implants are pre-filled with silicone gel. Silicone gel implants are FDA approved and are stable durable and cohesive. Cohesive means that it will not flow out of the implants shell, even if they rupture. Since they are inserted pre-filled, they require a slightly larger incision. This implant, many women believe, feels and looks very natural in your body. These implants are available for women over the age of 22.

SIZING FOR YOUR SAN DIEGO BREAST IMPLANTS

Determining a cup size is not a consistent measurement and may vary from one bra manufacturer to another. However,No surgeon is able to guarantee a specific cup size following  Breast Augmentation.

Therefore, determining which implant size is best for you will depend on several factors;

  1. Your desired look.
  2. Shape and size of the breasts,
  3. Your breast measurements.
  4. Type of implant you choose.

During your consultation bring with you picture of breast augmentation that you like and, in addition, ones you don’t like. This will give your surgeon information on what you expect. Using implant sizers and sometimes imaging will help you in determining the size of the implant. However, a big factor in determining the size of the implant is your anatomic measurements and the skin envelope.

Moreover, It is really helpful to you if you try to avoid using cup sizes as the goal for implant size. Bras come in different shapes and padding and the cup size will not be helpful in selecting the implant size.

Furthermore, Very large and heavy implants (for your specific anatomy) may not give you the desired look and potentially can “bottom out” overtime. Unpredictable stretching and thinning of your skin tissue over the years is a  risk with larger and heavier implants and may potentially lead to further surgeries, such as breast lift, and more scars.

Your surgeon will take measurements of your anatomy to determine the range in size of implants best suited for your body frame.

Can I have another cosmetic procedure with breast augmentation?

If you are looking to have more than just breast augmentation you can ask Dr. Jaibaji, MD about combined procedures. Breast augmentation is frequently combined with other procedures. These procedures include liposuction and fat transfer, nose job, also known as  rhinoplasty, tummy tuck or any other facial procedure.

I have had my implants for 10 years; do I need to replace them?

The answer depends i the status of the breast and implants and, in addition, on your goals.

If the implants are intact, no capsular contracture, no pain and no change is breast shape then you don’t need to replace them.

However, Implant replacement is necessary for the following reasons:

1- Implant rupture or deflation.

2- Capsular contracture with pain and or deformity of one or both breasts.

3- Breast ptosis ( sagging)

4- If you desire to have bigger or smaller implants or different implant type.Like changing from saline to silicone.

Breast Augmentation Recovery

The surgery usually takes 1-2 hours to complete. Most patient will leave the surgery center after about 1-2 hours in the recovery room. A family member or a friend will be with You after surgery and will drive you home.

A responsible adult must stay with you for the next 24- 48 hours. Even if you feel good after surgery you will not be a able to drive after surgery. Patients feel tired and sore following  Breast Augmentation surgery. This will usually pass in a day or two.

Many of our patients are able to return to work within the week following breast augmentation. Any after surgery pain, swelling and sensitivity will fade over the first few weeks. All of your sutures are dissolvable and your surgeon will see you the day following your surgery.

A good surgical technique will speed your recovery process. Therefore choosing your surgeon is an important first step to proper recovery.

Before and after breast Augmentation?

Your scars following breast augmentation will begin to fade in weeks and months after surgery  and will continue to improve over time.

Post breast augmentation activity?

We will provide you with special instructions on when to resume exercises and your normal activities. However, light activity is encouraged for the first two weeks following your procedure. On the other hand, No heavy lifting or strenuous activity should be done during the first 6 weeks after surgery. Full recovery may take six to eight weeks, at which time activities such as skiing, wakeboarding and contact sports can be undertaken safely. Listen to your surgeon’s recommendations.

Boob Augmentation… Did You Know?

Implant surgery can not only increase the size of the breasts but correct asymmetry between them. Breast augmentation can also correct breast sagging, to a certain extent, that develops through the natural aging process.

During the breast augmentation procedure,Dr. Jaibaji creates an incision either beneath the breast, around the areola, or under the arm, depending on what is best for the cosmetic goals of the patient. Breast augmentation pain

During you breast augmentation surgery a local anesthetic will be placed around the implant. This will provide pain relieve for 6-8 hours after the surgery. Following that you can take oral pain killers to control the pain. Most patient will experience some discomfort in the first day after surgery and then the pain is much less.

Placing the implant over the muscle will create less pain. However, that  pocket placement may increase the risks of capsular contracture.
